Compliance Reporting
All staff and applicable affiliates of Concern Housing have a responsibility to report issues which may constitute non-compliance with regulatory requirements (such as Medicaid Regulations, HIPAA Regulations, etc. ). As part of our Compliance Program, we offer means for staff to report fraud, unlawful or unethical behavior or any other violations of Concern’s Compliance Program.
Staff are encouraged to discuss any concerns with a supervisor to work toward identifying and correcting issues. If reports to a supervisor are ineffective or impractical, a report of suspected non-compliance can be made anonymously in one of the following ways:
- Website: lighthouse-services.com/concernhousing
- Toll-Free Telephone:
- English speaking USA and Canada: 833-750-0001
- Spanish speaking USA and Canada: 800-216-1288
- E-mail: reports@lighthouse-services.com (must include Concern for Independent Living as company name in report)
- Fax: (215) 689-3885 (must include Concern for Independent Living as company name in report)
- Mail: Concern for Independent Living, 312 Expressway Dr. S., Medford, NY 11763, Attn: QI Compliance Department.
These reporting methods are not a substitute for routine communications within our organization between employees and their supervisors and managers. Likewise, it does not replace communications with HR staff about benefit issues or other job related issues. These reporting options are additional communication tools for specific types of situations and are provided because we believe that it is a good business practice to do so.
We will not tolerate any retaliation against employees who raise genuine concerns. Such retaliation is cause for disciplinary action up to and including termination of employment.
If you have any questions, please reach out to Elizabeth Weinbaum, Senior Associate Executive Director or Catherine Fontana, Director of QI and Compliance at (631) 750-0474 or QICompliance@concernhousing.org.
