Scattered Site Supportive Housing
House
Concern's Supported Housing programs provide affordable, permanent housing and services for over 500 individuals and families, including families with children. These programs provide clients with assistance in choosing and maintaining the house in which they want to live. Clients live in houses or apartments and receive rental assistance, case management and property management services from Concern. Concern's staff coordinates additional services with other providers and meets with their clients regularly. Concern also operates a hotline for all of its residents, and staff are available in emergencies 24 hours a day. Concern has specialized supported housing programs for the homeless, families and survivors of trauma, as well as housing with enhanced Assertive Community Treatment (ACT) support services.

Single Site Supportive Housing
House
Concern operates 200 units of transitional, licensed housing, in approximately 20 sites throughout Suffolk County. Concern's Supervised Residences provide housing and support services for up to 12 individuals at each site. The residences are staffed 24 per day, 7 days per week. Services include training in cooking, cleaning, shopping, medication management, personal hygiene, money management, linkages with vocational and/or educational programs, and other essential living skills. Staff also provide case management and crisis intervention services as needed.

In C-TAP, residents live with one or two other individuals in a community setting. Staff work with clients on setting and attaining rehabilitative goals. Daily living skills training, case management, and crisis intervention are provided according to client needs. This program offers a greater degree of independence than the Supervised Community Residences while providing resident with supportive services they need to prepare for more permanent, independent housing in the future.

Concern has an array of housing options ranging from highly supervised community residences to independent housing in apartments and houses. Together with staff, referral sources and family members, clients choose the housing that best meets their needs and rehabilitative goal. Concern is committed to providing safe, affordable housing with supportive services in a culturally sensitive environment.
