Supported Housing

Concern’s Supported Housing programs provide affordable, permanent housing and services for over 350 individuals and 40 families with more than a hundred children. Program participants live in houses or apartments and receive rental assistance, case management and property management services from Concern. Concern's staff coordinate additional services with other providers and meet with their clients regularly. Concern also operates a hotline for all of its residents, and staff are available in emergencies 24 hours a day. Concern has scattered site supported housing programs for individuals and families throughout Suffolk County, Nassau County and New York City, and specialized programs for the homeless, survivors of trauma, and persons with substance abuse disorders.


 

Community Residences

Concern operates 38 units of transitional, NYS Office of Mental Health licensed community residence housing, in 4 sites in Suffolk County. Concern’s Supervised Residences provide housing and support services for up to 12 individuals at each site. The residences are staffed 24 hours per day, 7 days per week. Services include training in cooking, cleaning, shopping, medication management, personal hygiene, money management, linkages with vocational and/or educational programs, and other essential living skills. Staff also provide case management and crisis intervention services, as needed.


 

Apartment Treatment Residences

In Concern’s Apartment Treatment Program, residents live with one or two other individuals in a community setting where they are visited by staff 3-4 times per week. This program serves 30 individuals in 9 scattered sites in Suffolk County. Staff work with their clients on setting and attaining rehabilitative goals. Daily living skills training, case management, and crisis intervention are provided according to client needs. This program offers a greater degree of independence than the Supervised Community Residences while providing residents with supportive services they need to prepare for more permanent, independent housing in the future.



Single Site Supportive Housing

Concern operates five New York State Office of Mental Health licensed single site supportive housing programs in Suffolk County and Brooklyn. Residents each have their own studio apartment with a kitchenette, private bathroom and living area. In addition, these buildings offer a variety of community spaces, including computer rooms, lounges, fitness rooms, laundry facilities and dining rooms with commercial kitchens. Staff work with residents to further develop independent living skills through the provision of restorative services, including: medication & symptom management, budgeting, daily living skills training, etc. Residents have leases and participate in a Resident Advisory Council. As in all of Concern’s programs, our focus is on enhancing the wellness of the whole person.


Community Case Management

In addition to housing and housing related services, Concern provides contracted case management services for Long Island veterans with HUD-VASH Vouchers.  This program utilizes a Housing First approach to ensure rapid rehousing and stabilization within the community.  Concern Case Managers provide services be to assist in the expedited placement of HUD-VASH veterans and their families into permanent, supportive housing and to assist them in maintaining their housing and gaining as much independence as possible.  Services are coordinated with other medical, psychiatric, educational, and vocational providers. 


Mixed-Use Supportive Housing

Concern’s mixed-use housing program in Brooklyn provides a mix of studio, one-, and two-bedroom apartments.  Persons with disabilities are offered studio apartments with private kitchenette and bath and individualized support services.  One- and two-bedroom apartments are available for low-income individuals and families from the community.  Building amenities include physical fitness center, computer room/library, lounges, laundry facilities, outdoor recreational areas and 24/7 on-site security.
