Job Title:          Case Manager (2)
Hours:              9am – 5pm
Location:        Supported Housing
Days:                 Monday through Friday
Requirements: Bachelor’s degree in Psychology, Social Work or other related mental health discipline plus at least two years experience working with psychiatric population. This individual must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.

Job Title:          Entitlements Coordinator
Hours:              9am - 5pm
Location:           Medford Office
Days:                Monday - Friday
Requirements: Bachelors Degree and a minimum of two years experience working in a position involving entitlements/benefits in a social service agency.

Requirements:    Knowledge of mental health issues and some work experience in the mental health field required. Valid driver’s license.

Job Title:          Medication Manager
Hours:              3pm – 11pm
Location:          E. Patchogue SRO
Days:                Monday - Friday
Requirements: Bachelors Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years experience working with a psychiatric population. A working knowledge of psychotropic medications is also required. This individual must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.

Job Title:        Cook P/T
Hours:             Friday 4p-8p, Sat 12n-8p, Sun 8a-6p
Location:        E. Patchogue SRO
Days:               Friday, Saturday, Sunday
Requirements: High school diploma or equivalent. Professional cooking experience.
